Step-by-Step Instructions for How to Submit a Request - From the Web
Note: the video above and instructions below are meant to supplement the training you receive directly from your local network leaders
- Identify the need for a family you serve according to the training received from your local CarePortal network leaders and your own agency leaders
Note: permission from the family should be received before entering any new CarePortal request - From your CarePortal dashboard, under the "Request Management" section of the left-hand navigation menu, click on the "Submit Requests" button

From the CarePortal home page, click the "Submit Request" link in the top right of the page
Note: for new agency users, a new account can be created within the process of entering your first request from the CarePortal home page - Under the "Request Management" section of the left-hand navigation menu, click on the "Submit Requests" button
- Enter the Zip code of the family or child in need
- Confirm the county for this request (only shows up if Zip codes span multiple counties)
- Select the request tier for the request
- A larger screen will appear with the rest of the fields needed to complete this request, including:
- County of Caseworker
- Case Number or ID for family (not displayed publicly)
- Request Urgency Level
- Purpose of This Request
- Number of Children Served
- Number of Adults Served (if applicable)
- Role or Function of Caseworker
- Email of Caseworker Supervisor (if required by Agency Settings)
- Story or Description of this Request (NO CONFIDENTIAL INFORMATION) - At the bottom of the form, enter each item needed and confirm the estimated value for each item
- Click the Submit Request button to submit the request for approval. Once approved, CarePortal will notify the closest church response teams to the family and an email confirmation will be sent to the Agency Requester.
Note: Requests submitted by Agency Reps are automatically approved; Requests submitted by Agency Workers require the approval of a designated Supervisor or Agency Rep. - The Agency Requester will be notified via email when an approved church responder or leader has committed to items in their request.

Step-by-Step Instructions for How to Submit a Request - From the Mobile App
- Identify the need for a family and receive permission from them according to your agency's internal process to submit a CarePortal request on their behalf
- Click the + button
- Enter the Zip code of the family or child in need and confirm the county for this request
- Enter the case number, number of children and adults served, and demographic information
- Select the request tier for the request, role/function of the caseworker, and the purpose of this request
- Select the level of urgency of this request
- Click "ADD A NEED" to begin adding needs
- Select the item category, then the specific item. You can choose if it's for a girl, a boy, or doesn't matter, add the quantity needed, and adjust or confirm the estimated value
- Click "SAVE THIS NEED"
- Repeat steps 7-9 until you've added all the items needed for this request
- Click "Continue"
- Add the Story or Description of this Request (NO CONFIDENTIAL INFORMATION)
- Click the green check mark to submit the request
- Requests submitted by Agency Reps are automatically approved; Requests submitted by Agency Workers require the approval of a Supervisor or Agency Rep. Once approved, CarePortal will notify the closest church response teams to the family and an email confirmation will be sent to the Agency Requester.
