This video provides foundational training of how the CarePortal platform works for agency workers.

Step-by-Step Instructions for How to Submit a Request
Note: the video above and instructions below are meant to supplement the training you receive directly from your local network leaders
- Identify the need for a family you serve according to the training received from your local CarePortal network leaders and your own agency leaders
Note: permission from the family should be received before entering any new CarePortal request
- From your CarePortal dashboard, under the "Request Management" section of the left-hand navigation menu, click on the "Submit Requests" button
- Enter the Zip code of the family or child in need
- Confirm the county for this request (only shows up if Zip codes span multiple counties)
- Select/confirm the tier the request falls into (the vast majority of requesting partners are only approved to submit tier 1 requests)
- Click the "Continue" button and in the next step, complete each of the various fields:
- County of Caseworker
- Case Number or ID for family (not displayed publicly)
- Request Urgency Level
- Purpose Statement of the Request
- Number of Children Served
- Number of Adults Served (if applicable)
- Email of Caseworker Supervisor (if required by Agency Settings)
- Race of Children Served (not displayed publicly)
- Click the "Continue" button and in the next step, write a general description of the situation the family/children are in, without using any identifying or confidential information, so responders can get a sense of what is happening and why the requested items are needed.
- Use the "Add Need" button to select from the catalog of commonly requested items to add each individual need that you'd like responders to be able to commit to. Customize the quantity and estimated value for each item, as needed.
- Click the Submit Request button to submit the request for approval. Once approved, CarePortal will notify the closest church response teams to the family and an email confirmation will be sent to the Agency Requester.
Note: Requests submitted by Agency Reps are automatically approved; Requests submitted by Agency Workers require the approval of a designated Supervisor or Agency Rep.
- The Agency Requester will be notified via email when an approved church responder or leader has committed to items in their request.
