Step-by-step instructions for how to submit a request
Important! The video above and the instructions below are meant to supplement the training you receive from your local Network Leaders. You must have an Agency role with an enrolled Agency in CarePortal to submit a request. To learn more about Agency roles, visit the Understanding the Agency Roles Within CarePortal article.

1. Identify the need.
Identify a real need for a family or child you serve, based on:
- Your training from local CarePortal Network Leaders
- Guidance from your own Agency leaders
Important! Ensure you have permission from the family before entering a CarePortal request on their behalf.

2. Open the Submit Request page.
Open the navigation menu, go to Request Management, and select Submit Request.
3. Enter the family's zip code.
Type in the zip code where the child or family lives.
4. Confirm the county.
If the zip code covers more than one county, you'll see a county confirmation step. Confirm the correct county for this request.

Important! You won't be able to submit a request if:
- The family's zip code isn't in an approved county for your Agency or CarePortal is not yet active in that county.
- Your Agency has reached the county's monthly request limit (if one is set).
Reach out to your CarePortal point of contact if this happens.

5. Select the request tier.
Choose the tier that best matches the need. Most requesting partners are only approved to submit Tier 1 Requests.
6. Select Next and complete the required fields.
On the next screen, fill out each of the following fields:
- County of Caseworker
- Case Number or ID for the family (not displayed publicly)
- Request Urgency Level
- Purpose Statement of the Request
- Number of Children Served
- Number of Adults Served (if applicable)
- Email of Caseworker Supervisor (if required by Agency Settings)
- Language(s) the family speaks (defaulted to English β select all that apply)
- Race of Children Served (not displayed publicly)
- Is the family Hispanic-Latino (not displayed publicly)
7. Select Next and describe the situation.
Write a short, clear description of the family's situation. Focus on why the items are needed and how they will help. Do not include any identifying or confidential information.
8. Add specific needs.
Select Add Need and use the catalog of commonly requested items to add each individual need you want Responders to commit to. Adjust the quantity and estimated value for each item as needed.
9. Submit the request for approval.
Select Submit Request. Your request will be routed according to your Agency's approval settings. Once approved, CarePortal will notify the closest Church Response Teams to the family. The Agency Worker or Rep who submitted the request will receive an email confirmation.
Tip: Requests submitted by Agency Reps are automatically approved. Requests submitted by Agency Workers require approval from a designated Supervisor or Agency Rep before CarePortal shares them with Churches.

10. Watch for responses.
The Agency Worker or Rep who submitted the request will receive an email when a Church Response Team member commits to items in the request.
