Add Location Field to Response Form

Feature Request Summary: 

As a responder confirming my response, I should be able to confirm the location (city, state) of the items I am providing/funding as well so that I can truly confirm I am able to fulfill the request.

What:

Add a field to the response form for Responders to add their location.

Why:

This would be beneficial for Churches that are helping fulfill a request to know where the responder is located. Especially if an item is out of town/state.

1. Add "Location of this request" to the confirmation message as shown in mock-ups (match new text to "Total amount being funded")
