Agency User Activity Tracking

What is the problem?

As an agency admin, I use CarePortal to pull data on how much my workers and agency reps are using CarePortal so that I can include that information in their monthly performance evaluations, but I don't have a clear or easy way to get that information within CarePortal. 

What outcome am I looking for?

I would like to see, for each user:

1. How many requests they've submitted
2. How many of those requests were declined
3. How many requests they've approved
4. How many requests they've declined

And I would like to be able to filter by date so the numbers are displayed based on the date range.

This way I can see how much each user is using CarePortal.
